Identifying and Assessing Your Culture Needs

It is time to get to the hearts of your residents, families and staff. This survey will be an internal and external assessment of what is truly important to all. It is important to get as many people as possible to participate.

Distribute the survey to all of your staff members, mail it to families, hand it out at resident/family council meetings. Ask a Culture Change Team member to submit a request to explain the purpose of the survey at a resident council meeting.

Make certain that all staff members understand the sincerity of the survey and the benefit of knowing what is important to the residents. Encourage staff to help the residents complete the survey.

A sample Nursing Home Growth Survey is included. The survey was created in a Word document, so you use it as is or modify it to meet the needs of your building.

The Personal Preferences Questionnaire was developed by the Culture Change Pilot Group as a way of getting to know the personal preferences of each resident. This is a great tool to incorporate into the process for admitting new residents. Have your staff review the questionnaire and think about the individual residents currently in their care. If they don’t know the residents’ preferences, encourage them to take the time to find out.
